SHOW BOOKING FORM

After selecting a date with the Events Coordinator, YOU HAVE ONE (1) WEEK to complete and submit this form. If the form is not received within one week, your selected date will be released. You will receive an email once your show has been officially booked.

Show titles/themes CANNOT include or reference any trademarked or copyrighted entity

(i.e. Disney, movies, song titles, brandnames, catchphrases, taglines).

Deadline for advertising elements is 8 weeks from show date. If this deadline is missed, design of your promotion will be completed as the production schedule allows.
